Case Manager (Hope Gardens)

Description:POSITION SUMMARY:To develop and facilitate the essential case plan components for single mothers at Hope Gardens Family Center.

Work involves assessments, interventions, individual goal setting and spiritual encouragement.

The goal for each employee is to enrich and sustain a humble heart attitude; to enrich and sustain amindset of serving others, treating them with kindness and truth; and to enrich and sustain a zealous desire for achieving work objectives.ESSENTIAL FUNCTIONS:Has a passion for and ability to share the love of Jesus Christ with others.Able to develop a clearly written case management plan for services, objectives, and individual goals for assigned caseload.Perform case management activities which include intake assessment and evaluation.

Emergency and special service needs related to legal, medical, or mental health issues.

Following our core model, able to track guests progress through the program and willing to keep them accountable to their own goals.Meets with every guest assigned to their case load as often as the specific Program Phase their guest is in requires.

(1st week meets with new intakes every day, Phase 1& 2 once a week, phase 3& 4 once every 2 weeks etc.).Able to refer guests to essential services such as housing, county benefits, education etc.

as mapped out in their case plan and at the appropriate time of the program.Maintain guest records and document weekly and monthly-related case management documentation into URM database.Compose and process various types of correspondence to include letters, sensitive and confidential information, official and/or legal letters and documents.Assist in supervising volunteers, as needed.Travel to meet accountabilities, as required.Arrange guest transportation, as needed.Carry other tasks and projects as assigned by the Programs Manager.Commitment to URM mission, vision, and core values.Encourage guests of their faith and growth in Jesus Christ.PHYSICAL DEMANDS:In general, the following physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job.

· The office and classrooms are clean, orderly, properly lighted, and ventilated.PM20Requirements:EXPERIENCE, EDUCATION AND LICENSURES:Must have High School Diploma or GED.Bachelor’s degree in behavioral sciences, social work, or a closely related field preferred.Minimum 3 year of prior related experience.Prior experience working with or serving families experiencing homelessness or with populations who are marginalized, preferred.Valid driver’s license with the ability to be added to the company’s insurance policy.KNOWLEDGE, SKILLS AND ABILITIES:
· Must have excellent time management skills and ability to multi-task in a high stress environment.
· Ability to communicate effectively in both written and verbal formats.
· Proficient in Microsoft programs including Word, Excel and Outlook and the ability to learn proprietary computer programs.
· Fluency in Spanish, both verbal and written, is a plus.
· Child Development and Special Education background is a plus.
