Job Details Level Experienced Job Location Main Hospital – Los Angeles, CA Remote Type N/A Position Type Undisclosed Education Level High School Salary Range Undisclosed Travel Percentage Undisclosed Job Shift Day Job Category Health Care Description Case Manager (GCAOP) Gateways Hospital & Mental Health Center The ICC Case Manager will be responsible for collaboratively assessing resources and needs, making referrals and linking to services, and monitoring/following up to ensure that the client and family’s needs are met. DUTIES AND RESPONSIBILITIES Read, thoroughly understand, and comply with all policy and procedures, including: DMH Manual, LAUSD Requirements, and Gateways Adolescent Outpatient Staff Handbook. Participate in CFT and/or school Meetings. Provide services in the community, home, telehealth, or wherever the family feels most comfortable receiving services; must be flexible regarding time and place of meetings with client and family. Provide case management linkage services to client and family as identified during CFT and/or school Meetings. Provide rehabilitation services which include assistance in improving, maintaining, or restoring one or more functional skills, daily living skills, social and leisure skills, grooming and personal hygiene skills, and support resources; and/or medication education. Work collaboratively with the multi-disciplinary treatment team. Communicate and interact with client and family from a strength-based, collaborative, and supportive stance. Knowledgeable about government programs, such as SSI, MediCal, etc. Maintain active and effective liaisons with relevant community agencies to foster linkages. Complete all required documentation in a timely manner, as specified in the L.A. County Short Doyle/Medi-Cal Manual, including completing progress notes within 24 hours. Input documentation into the Electronic Health Record system. Attend trainings to maintain technical skills and foster professional development. Attend and participate in weekly individual supervision. Case coordinator will have on call responsibilities when crises arise Responds appropriately to emergency situations and exercises good judgment by objectively and unemotionally analyzing facts, weighing evidence, reaching sound conclusions, and asking for support as needed. Perform other additional related tasks as directed. Qualifications EXPERIENCE/QUALIFICATIONS: Prior experience working with children and families from diverse ethnics, cultural, and socioeconomic backgrounds. 2 years working with seriously emotionally disturbed youth and their families to provide Targeted Case Management. Experience participating in multidisciplinary teams. History of strong writing ability in previous employment. Bilingual (English and Spanish); demonstrate ability to fluently communicate in Spanish. ESSENTIAL SKILLS: Ability to work and relate effectively with multidisciplinary treatment team, clients, and public. Ability to communicate effectively in English and Spanish, both verbally and in writing. Ability to remain calm, objective and dependable in situations where clients are emotional or uncooperative, and situations where there are assignments that must be met. Ability to appropriately follow instructions and feedback from supervisors. Functional experience with Los Angeles County Mental Health System. Be able to meet service delivery expectations weekly. Ability to sit for periods of 2 hours. Experience with an electronic health record system Knowledge of basic computer skills, including Microsoft office, email communication, video conferencing, and typing. EDUCATION: Minimum Education Required: High School Diploma with at least 5 years experience with target population providing Case Management and Linkage services to youth and their families; or B.A. or B.S. in mental health related field and two-year experience with S.E.D. children in a hospital, group home, day treatment, classroom or other equivalent setting; or 2 years of college plus 3 years of experience with S.E.D. children in a hospital, group home, day treatment, class room or other equivalent setting. REQUIREMENTS Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI) background clearance. TB clearance. Fire and Safety Training. Valid California drivers license Driving record acceptable for coverage by Gateways Hospital insurance carrier Note: If indicated certifications/licenses are lacking, they must be obtained within 180 days of hire. PHYSICAL REQUIREMENTS To perform this job you must be able to carry out all essential functions successfully.
