Case Manager – Family Advocate

The Child and Family Guidance Center provides comprehensive and collaborative outpatient mental health services to children, adolescents and their families throughout the San Fernando and Antelope Valleys.

The Position:

We are seeking a full-time Bilingual Family Advocate to assist families with locating, accessing and effectively using essential community health and social services.

Job Requirements:

Essential Job Functions:

  • Conducts in home psychosocial assessment of the family’s strengths and needs and develops a case management treatment plan based on the parent’s goals and in coordination with the treatment team.
  • Works closely with the child’s school when their mental health symptoms impact their academic performance. This includes, obtaining and reviewing school records and IEPs, conducting observations in the classroom and educating parents about their child’s educational rights.
  • Assists parents with increasing their understanding of their child’s special needs and knowledge of successful techniques for managing their child’s mental health symptoms in the home and community.
  • Coordinates with community agencies to locate organizations that offer adequate and culturally sensitive services requested by the family.
  • Completes clinical documentation in a timely manner and in accordance with the standards set by the Los Angeles County Department of Mental Health

Skills You Need:

  • Bilingual in Spanish and English
  • Minimum of 2 years experience providing case management services to children and adolescents.
  • Experience with locating support services for low income children and families.

Hours:

  • 8:30 a.m. – 5:00 p.m. Monday-Friday (subject to change depending on agency needs)
  • 30 minute meal period

Education Required:

  • BA/BS in Social work or related field
