– Roseburg, OR Position Type Full Time Education Level 4 Year Degree Job Shift Day Description POSITION PURPOSE: The Case Manager assists in the development and maintenance of a healthy, supportive environment to make provision of effective therapeutic interventions possible within the community.

The Case Manager works with individuals diagnosed with mental illness to promote recovery, independence and successful community living, by or through (a) Communication of hope, and promotion of emotional, behavioral and psychological growth through persistent efforts to attain individual goals; (b) The promotion of skills and knowledge to help individuals effectively manage their mental health concerns and develop a sense of hope and a sense of self that is not illness dominated; and (c) Provision of a humane service environment that affords reasonable protection from harm including re-traumatization.

Provides case management, skills training and mentoring services to individuals with mental illness within the community and outside of COMPASS BEHAVIORAL HEALTH offices.

Provision of treatment services are informed by the Individual Service and Support Plan (ISSP) and clinically supervised by the primary clinician.

Services are provided within a program for the Mental Health Division of the Health and Social Services Department in consultation with the treatment team.

This classification provides advocacy and linkage in planning, monitoring, coordination, and implementation of the treatment interventions as identified in the ISSP.

In addition, Case Manager’s assist individuals in problem-solving to reduce barriers to recovery and support the development of resources within the community while coordinating services with other community and treatment providers.

Case Manager’s provide a variety of treatment services including individual and group skills training and development; case management services; coordination and monitoring daily structured activities of clients; performs crisis intervention as necessary under the guidance of a therapist (QMHP) and/or supervisor; provides consultation and training; facilitates communication and correspondence with other COMPASS BEHAVIORAL HEALTH staff, community partners, other treatment providers, peers, and all others as needed.

This classification coordinates and collaborates with a multi-disciplinary treatment team and is responsible for work performed in assigned specialized program area.

This position will provide direct billable services that meet the current standards set by the Mental Health Director.

The employee will adhere to all applicable COMPASS BEHAVIORAL HEALTH ethics policies, procedures and guide lines including but not limited to HIPAA requirements.

ESSENTIAL JOB RESPONSIBILITIES: Provides case management, individual and group skills training and development, mentoring and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP); uses interventions informed by Evidence-based Practices endorsed by Compass Behavioral Health.

Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving with the treatment team and foster providers about outcomes, barriers and case observations.

Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.

As many of the services are provided with in the community and throughout Douglas County, driving is an essential and daily function of this position; transports individuals, family members and other supports to/from services Participates in professional development through individual and group supervision, unit and agency staff meetings, workshops and Essential Learning tutorials; develops, modifies, and or obtains training programs and materials in consultation with supervisor and program manager; promotes peer professional development by sharing training materials and or training others in successful interventions.

Unique duties maybe assigned to staff members by their Program Manager or Supervisor, to meet agency needs; current incumbent in this position supports 370 program clients and works with law enforcement, the State Hospital and case managers to coordinate their re-entry into communities.

Emergency Response: This employee has been identified as a staff member expected to report for duty following activation of a public health emergency response plan.

Qualifications KNOWLEDGE/SKILLS/EXPERIENCE REQUIRED: Bachelor’s degree and preferred two years’ experience in a social service and/or mental health setting or a satisfactory equivalent combination of education, experience and training.

At time of appointment, must possess valid Oregon driver’s license.

Knowledge of basic psychological theory, principles and practices of mental health treatment including assessment, diagnosis and treatment of mentally ill persons; human behavior as it relates to crisis situations and problems of client population; substance abuse treatment as it pertains to the mentally ill, considerable knowledge of behavior intervention, management and support strategies; sound case management; knowledge of community resources including the functions and procedures of other social service agencies and service providers; education and ability to develop skills groups and workshops including planning, implementation, and evaluation of outcomes; reasonable knowledge of educational theories and techniques; reasonable knowledge of psychoactive medication and treatment applications.

Skill in communications that promote teamwork and collaboration with individuals served; written reports and correspondence that is both professional and informative; documentation that is clear and concise; use of motivational interviewing and other communication techniques and principles; an understanding of individual and group dynamics.

Ability to communicate effectively in both written and oral form; understand how the assessment and Clinical Formulation informs the ISSP and how the ISSP identifies and prescribes specific interventions; plan, consult and coordinate with source agencies, physicians, families of clients and the community; work within inter-disciplinary team; relate to a diverse population of people with mental and physical challenges; make decisions in accordance with established policies and procedures, use initiative and judgment in completing tasks and responsibilities; utilize problem-solving skills to assist individuals in being able to reduce barriers to treatment and recovery; adapt to the challenging needs of the agency and the population served; utilize problem identification and resolution techniques, work independently and in collaboration with team, efficiently organize time and meet established deadlines; remain calm and use good judgment during confrontational or high pressure situations; teach socialization and living skills to individuals with a variety of challlenges; establish and maintain records, reports and statistical data utilizing the electronic medical record system; and approach others including individuals served and their natural supports, other staff and community partners, and the individual’s treatment team with courtesy and a cooperative attitude.

Work is performed under the general direction of a Qualified Mental Health Professional (QMHP) and with the clinical supervision of a supervisor or program manager.

Recurring routine assignments are independently performed by Case Manager on basis of past experience.

The Case Manager receives general instructions regarding the scope and approach to projects or assignments, but is able to ask for and obtain specific procedures and problem resolution strategies when needed.

Employee estimates and manages time efficiently.

Work is subject to active supervisory control and review through regularly scheduled supervision and meetings to ensure determinations and decisions made are in compliance with departmental goals, policies, and procedures.

Supervision is not a routine responsibility of this position, however, incumbent is expected to assist in the training and support of co-workers and internal and external customers.

Work requires on-going interaction, effective communication, and active collaboration with individuals and families receiving treatment services through COMPASS BEHAVIORAL HEALTH, natural and community supports, agency partners, and other treatment providers including staff at COMPASS BEHAVIORAL HEALTH.

Contact occurs through various means including in-person, by telephone or correspondence, and in coordination of care with other providers.

Contact also includes provision of psycho education for treatment and prevention, and outreach efforts to specific individuals and the general public.

PHYSICAL/MENTAL ABILITIES AND PROCESSES: Regular and consistent attendance is required.

Work is performed in a variety of environments including facility, community, and within the home.

Work may involve bending, hearing voice conversation, keyboarding, lifting up to 34 pounds, sitting, standing, walking, and driving.

Considerable attention is given to personal safety when in contact with potentially aggressive clients.

Employees must maintain a positive mental attitude and a tolerance for pressure situations.

Position requires driving a vehicle.
