As part of our RRH Team, Case Managers work in the field alongside other case managers, housing locators, and community providers to provide outreach, linkage, and supportive services to individuals experiencing homelessness to assist them in obtaining and maintaining stable housing Responsibilities of the Case Manager include: Collaborating with clients to develop individualized treatment plans and monitor progress Providing strength-based case management and service coordination designed to assist clients in obtaining and maintaining stable housing Providing field-based case management services such as coaching on independent living skills, assistance with housing stabilization and benefits establishment, and linkage to community providers for services needed by clients to reach their goals Providing services focused on enhancing clients?
ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate care Performing housing location and retention services by conducting neighborhood canvasing, landlord outreach, unit inspections and providing support to resolve disputes between land lords and tenants Working closely with program employment specialists to assist clients in obtaining and maintaining employment AppleOne is proud to be an Equal Opportunity Employer.
