What you will do
Have a unique opportunity to help to grow our business and impact the bottom line!
Responsible for identifying appropriate accounts/patients through review and consultation with health care professionals to assist in securing appropriate services and follow-up support
Helps to achieve and maintain continuity of patient care by planning and exchanging information with other professionals within the hospital, long term care facilities, physician offices, medical groups, case management offices and others in the community
Develops and maintains relationships with, and identifies needs of community organizations, utilization review nurses, discharge planners, case managers, physicians, hospitals, nursing facilities, medical groups, and other health care professionals who have an opportunity to utilize Agency services
Acts as a liaison between the Agency and the physician/facility/hospital with regard to patient care and education when necessary
Attends and participates in rehabilitation, discharge planning and patient care meetings as requested
Provides and/or coordinates agency sponsored educational opportunities and presentations within the community
Promotes positive working relationships with referral sources through effective communication and problem solving strategies to help facilitate appropriate service delivery
Responsible for achieving monthly goals as assigned in the individual Annual Sales Commission plan on a consistent basis

Who you are:
High School Diploma is required; Associates Degree or higher preferred
At least 2 years of client service and sale experience, health care industry experience is preferred
Proven ability to achieve sales goals
Must possess excellent organizational skills and be detail oriented
Must demonstrate strong written and verbal communication skills and be able to interact effectively with health care professionals, patients, peers and management
