Pay/Hours:
- Pay Rate: $43.00 to $45.00per hour (possible pay increase after 90 days)
- Hours: Full time – Monday – Friday 40 hour 8:00 a.m. – 5:30 p.m.
Job Description:.
- Purpose of the position: Case Management is “de-delegating”, meaning that provider groups are no longer doing case management on the members. These temps will take that on
- Workers will be given a larger case load (100-125) as “long term case management”. Will be managing all members over time. They will not be discharging members. Workers will make calls to members based on acuity and needs. They will be handling post discharge members.
Job Requirements:
Requirements:
- Associates degree in related field or equivalent experience. Bachelor degree preferred. 4+ years of nursing,
- Working knowledge of peer review criteria, NCQA, CMS, and other regulatory and accreditation requirements
- Prior case management experience with a health plan
- RNs with experience with Geriatrics (Elderly population) and members with acute needs
- CA Registered Nurse License
