Responsibilities
In this position you will support Pediatric Endocrinology in their CCS Special Care Center.
The primary goal of this position is to organize Center Care services within Pediatrics.
You will provide direct patient care and coordinate Endocrinology and Diabetes Center Care team clinics in the Children’s Health Center.
You will coordinate patient/caregiver teaching, referrals and follow-up nursing care for CCS Endocrinology and Diabetes patients; work with the team social worker to assist CCS Center Care-eligible patients to obtain coverage; track patient compliance with visits and medical care recommendations; prepare and conduct monthly team meetings for quarterly, biannual and annual reviews of Center Care patients, prepare and submit summaries of these meetings to CCS; communicate with patients, parents, and guardians; assist Center Care physicians, dietitians, social workers and other healthcare team members to optimize the efficiency, performance, and cost-effectiveness of Center Care services; and assist with the conduct of satellite Center Care services at outreach sites.
Qualifications
BSN or MSN, required
California RN License, required
BSL from the ARC or AHA, required
Previous Pediatric RN experience, highly desired
