THE POSITION

Under the supervision of the Program Manager, the ROS is responsible for planning, coordinating, implementing, and evaluating patient retention activities to reengage patients living with HIV/AIDS (PLWHA) within an HIV Medical Home. As a member of the Medical Care Coordination program team, the ROS collaborates to with MCC team members to identify patients who are eligible for MCC services. The ROS then contacts patients and provides brief, structured interventions to engage patients in care.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: (This list may not include all the duties assigned.)

  • Works with the MCC team to identify, locate, and re-engage patients back into HIV care.
  • Schedules follow-up appointments and conducts follow-up activities to ensure patients return for follow-up visits.
  • Assists in the coordination and delivery of patient-centered activities that focus on addressing health status, engagement and retention in care, adherence to HIV medications, and HIV risk reduction.
  • Works independently conducting office and field investigations (site visits to patients’ homes, neighborhoods, shelters, etc.) to locate, assess, and refer individuals for necessary medical evaluation and treatment.
  • Networks with community agencies and organizations to establish referral linkages.
  • Contact the case worker to assist in the determination of need and/or eligibility for services.
  • Tracks engagement and retention in medical care.
  • Participates in planning, training, evaluation meetings, quality assurance meetings, and conference calls, including travel as needed for training.
  • Maintains confidentiality of protected health information in compliance with state and federal law
  • Acts as the first point of contact with newly diagnosed patients to link them to care and MCC services.
  • Participates in staff meetings, Medical Care Coordination meetings, case conferencing, and other meetings as required.
  • Enters and maintains program data, including clinical documentation and reporting forms.
  • Enters monthly services data in Casewatch in a timely manner and prepares other demographic and statistical reports as required by the funding agency and Program Manager.
  • Documents all services provided according to standards set forth in the County of Los Angeles-Division of HIV and STD Programs (DHSP) funding contracts.
  • Other duties may be assigned, as required.

QUALIFICATIONS:

  • A Bachelor’s degree in public health, social work, psychology, counseling, nursing, human services, or health education OR A minimum of two years conducting outreach to vulnerable populations with the aim of improving health outcomes.
  • Experience and specific training/experience working with people living with HIV/AIDS preferred.
  • Proficient in MS Word and Excel software and tech-savvy.
  • Bilingual in English and Spanish is required.

COMPANY REQUIREMENTS:

  • Must be able to pass a background check to include a 7-year criminal, 10-year SSN & employer history reference check.
  • Excellent interpersonal skills.
  • Attention to detail.
  • Must be able to work flexible schedules.
  • Must have skills in research and development of operating policies and procedures.
  • Must take the yearly flu shot and test for tuberculosis as required by the Centers for Disease Control and Prevention.

LANGUAGE SKILLS:

Must be able to read, write and speak the English language fluently. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must have the ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public in person.

MATHEMATICAL SKILLS:

Ability to calculate figures and amounts such as dollar totals, monetary conversions, discounts, interest, fractions, decimals, percentages, area, and volume.

REASONING ABILITY:

Ability to solve technical and practical problems and deal with a variety of intangibles in situations where only limited communication, support, information and access exist. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

COMPUTER SKILLS:

Must have good computer skills and be proficient in the use of systems to complete correspondence, documents, spreadsheets, and presentations, such as Microsoft Outlook and Office; Word, Excel and PowerPoint, and Adobe Acrobat.
