Job Summary
Under the supervision of the Director of Quality Improvement, the Quality Improvement (QI) Specialist completes case record reviews for agency charts on an ongoing basis.
The QI Specialist trains staff in accurate chart documentation and provides additional support if needed.
He or she meets with staff on an ongoing basis to ensure accurate completion of chart documentation.
Essential Functions
Complete case record reviews in a thorough and timely manner for current staff caseloads, departing staff, and for audits on a ongoing basis Contact and meet with staff on an ongoing basis to ensure case record review corrections are completed in an accurate and timely manner Regularly meet predetermined goals for case record reviews and audit reviews Update, maintain, and submit documentation in a thorough and timely manner (i.e., Case Record Reviews, WiFi documentation, status lists, audit tools, DMH documentation and internal forms) Attend trainings, department meetings, and field based meetings as necessary; schedule and conduct WiFi interviews if applicable Train staff on chart documentation and provide additional support to staff who have difficulty in accurately completing chart documentation Open and close the chart room and file charts as needed Travel between all agency locations to conduct WiFi interviews if applicable Sensitive and responsive to the cultural differences among the agency’s client population Actively participate in the quality improvement project
The QI Specialist will take on special projects or additional duties as assigned by the Director of Quality Improvement.

Ability to speak, read, write and understand English Ability to work effectively as a member of a multi-disciplinary team Ability to constructively utilize supervision and consultation Must have excellent oral and written communication skills Must demonstrate high attention to detail Knowledge of Council on Accreditation guidelines and DCFS and DMH regulations Must have strong computer skills (Microsoft Word, Excel and Access) and be able to independently operate a computer
Education and Experience
Bachelor’s degree in Social Work, Psychology or a related field preferred and 2 to 3 years of relevant experience Experience auditing charts in an outpatient or residential setting preferred Experience with Department of Mental Health regulations and activity codes Experience creating and managing databases
Certifications/Licenses
Valid CA driver’s license

Tuberculosis clearance Department of Justice/Fingerprint clearance Satisfactory completion of health screening Proof of legal eligibility to work in the United States Satisfactory driving record

Physical Demands
While performing the duties of a QI Specialist, the employee is frequently required to sit, reach, drive and use his/her fingers to manipulate a keyboard or writing instrument.
The employee is occasionally required to walk and lift objects heavier than ten pounds.
The employee is rarely required to kneel.
Work Environment
The work environment is primarily indoors.
The noise level varies from quiet to moderate.
