Visitdignityhealth.org/northridgehospital (https://www.dignityhealth.org/socal/locations/northridgehospital) for more information.**Responsibilities****PROGRAM COORDINATOR****Grant-funded position**Northridge Hospital Medical CenterUnder the direction of the Director of the Center for Healthier Communities, the Program Coordinator is responsible for coordinating, supervising, implementing and evaluating assigned programs and activities for the Center for Healthier Communities with a focus on community health improvement and safety.Responsibilities to include:+ Oversees and implements the Local Elder Abuse Prevention Enhanced Multi-Disciplinary Team (LEAP EMDT).+ Develops effective outreach strategies for assigned LEAP program.+ Evaluates effectiveness and efficiency of programs.+ Represents CHC before community based organizations, governmental and policy organizations.**Qualifications**+ Experience implementing community based programs, developing outreach strategies, collaborating with community-based organizations, supervising, grant writing and grant management.+ Experience working with elders and knowledge of resources.+ BS/BA degree required.+ Chronic Disease Self-Management Program certified.+ Bilingual Spanish required.+ Must maintain current Fire Safety card.**Equal Opportunity**CommonSpirit Health™ is an Equal Opportunity/Affirmative Action employer committed toa diverse and inclusive workforce.
