Patient Navigator II

POSITION SUMMARY Navigators will respond to referral sources from government and non-profit organizations to include county agencies, hospitals, medical groups, health centers, mental health, and substance use treatment providers and others, and navigate patients to receive services at Tarzana Treatment Centers. HOURS 8 Per Day / 5 Days Per Week SUMMARY OF RESPONSIBILITIES Position is responsible for completing preliminary screening to help determine level of care and services to support recovery efforts, as well to help individual access needed health and social support services. Position conducts on-site and/or field- based outreach, engagement, re-engagement and navigation services. SPECIFIC DUTIES 1. Conduct initial navigation screening on the phone or in person with the objective to help determining the need and services of each individual and respond to referral sources from organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others. 2. Provide enhanced patient navigation services to identify and link eligible participants to project services. 3. Conducts follow up with individuals to ensure all scheduled appointments and linkages are completed. 4. Maintain accurate and timely Access notes to document all contact and navigation encounters with individuals. 5. Develops and submits accurate reports regarding program activities, clients served, caseload, retention rates, and other information as needed. 6. Thorough knowledge of program’s reporting database and can generate needed reports when assigned by Supervisor/Director. 7. Utilizes independence and professionalism within outside entities or client’s home when attempting to bring a client back into care. 8. Assigned to attend community meetings, Tasks Forces, and will represent TTC appropriately. Patient Navigator II w ritten 02/02/17 rev. 11/01/19 Page 2 9. Participate in outreach efforts at community sites and in-reach at TTC service site. 10. Maintain a database of patients served and outcome data for reporting purposes. 11. Attend agency and community meetings as directed by supervisor. 12. Additional tasks, projects and responsibilities as assigned by supervisor. 13. Able to act in a leadership capacity when assigned by Supervisor/Director. MINIMUM QUALIFICATONS KNOWLEDGE AND SKILLS: ▪ Passion and desire to work with individuals/families. ▪ Demonstrate a non-judgmental attitude and positive regard for families impacted by lack of access to health services. ▪ Experience providing field-based services. ▪ Strong people skills and ability to work with people of various ages, racial, ethnic, cultural, educational and socio-economic backgrounds. ▪ Organized, detail oriented and creative problem-solving. ▪ Proficient in computer skills and use of Microsoft products such as Outlook, Word and Excel. ▪ Must have valid CA driver’s license. EDUCATION/EXPERIENCE ▪ At least one-year experience as a Patient Navigator, or ▪ Minimum of 2 years of similar work experience such as Case Management or Benefit Specialty, or ▪ Registration or Certification as a Substance Use Disorder Counselor, or ▪ BA degree in SUD Studies, Psychology, or related field. ▪ Bilingual English/Spanish preferred SETTING Tarzana Treatment Centers, Inc. is a non-profit corporation, which provides behavioral healthcare services, including chemical dependency.
