Minimum 2 years of recent CDL experience driving similar equipment.
No more than 5 minor violations in the last 3 years (prohibited if all 5 are in last 24 months and submit if 3 are in the last 12 months).
No more than 1 major violation in the last 3 years per account (prohibited if 26 years old or younger and submit if less than 24 months old).
No more than 2 chargeable accidents in the last 5 years (submit if both are in last 12 months).
