Requirements:
- Updated Resume
- LVN License in State Applying for unless compact state.
- All Certifications depending what specialty you work in.
- 1-year min. LVN Experience in your current specialty
- Must clear background
- Must have clean licensure
- 2- Supervisory References
- TB – within 1 year
- Physical – within 1 year
- MMR Titers or 2 vaccinations
- Varicella Titer
- HepB series
- TDAP
