This role will be based out of our regional headquarters in Los Angeles and support LA County’s Recovery Rehousing Program in the Metro LA area. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. PATHs Housing Partnerships Program (HPP) provides landlord outreach and engagement services to develop a full range of permanent housing opportunities for individuals and families experiencing homelessness. The Recovery Rehousing program is part of LA Countys COVID-19 response and focuses on assisting those experiencing homelessness who are at a high risk for COVID-19 or have a high level of acuity and need in obtaining permanent housing.WHAT IS A LEASEUP COORDINATOR? As part of the HPP team, the LeaseUp Coordinator plays a key role in connecting participants in the Recovery Rehousing program with sustainable housing opportunities by facilitating coordination between the LeaseUp LA Program, PATH, and community partners. Responsibilities of the LeaseUp Coordinator include: Conducting daily outreach to assigned community partners to develop relationships with case managers and navigators to assist them with finding a home for their clientsAttending and representing the LeaseUp program at assigned Housing Surge calls with Recovery Rehousing providersAttending and representing the LeaseUp program at assigned Coordinated Entry System (CES) meetingsIdentifying appropriate housing opportunities for participants based on their needs and available housing in the LeaseUp portfolio and facilitating the matching process with case managersTraining case managers and navigators on use of the LeaseUp websiteAssisting with applications for housing opportunities and ensuring a timely follow-up for responses from the HPP teamMaintaining a high-level of responsiveness to inquiries regarding housing opportunitiesEnsuring requests for support from community partners receive responses and appropriate follow-up information within 24 hoursWHAT YOU BRINGWere looking for candidates that: Demonstrate knowledge and experience with advanced and evidence-based interventions including: Engagement and rapport building with hard-to-reach clientsHarm Reduction and Motivational Interviewing strategiesHousing FirstNon-violent crisis intervention techniquesCase management interventions proven effective with vulnerable populationsMaintain and execute confidential information according to HIPAA standardsDemonstrate strong problem solving and conflict resolution skillsEffectively use technology as a work tool including demonstrated proficiency with Microsoft Office Suite and database softwareWork just as well independently as they do on a teamExercise mature judgement, and are highly motivated, self-starting and proactiveAre excellent at communicating, whether in writing or verballyHave a strong sense of prioritization and can coordinate multiple demands in a high-pressure environmentMANDATORY REQUIREMENTSFor this role the LeaseUp Coordinator must: Hold a Bachelors Degree in a related field OR have at least 2 years of relevant experience working in homeless services, mental health, substance use and co-occurring disorders, or with vulnerable populations providing intensive case management servicesBe able and willing to work flexible hours which may include evenings or weekendsHave employment eligibility verificationHave or be able and willing to obtain CPR/First Aid trainingHave reliable transportation and: A valid drivers licenseProof of insurance and ownership for personal vehicles used during work dutiesThe ability to qualify for PATH insurance coverageSuccessfully complete the following as a condition of hire: Tuberculosis TestBackground ScreeningDrug TestREADY TO MAKE A DIFFERENCE THROUGH ACTION?If this sounds like you, please visit our PATH Careers Site (at and search for JR# 2048 to submit your application.
