Lead CNA-Personal Care Attendant III

Overview: Under the direct supervision of the PACE Supervisor, the PCA III oversees and coordinates all PCA’s work and ensures that they are in compliance with all licensed providers and other staff members’ orders.

In the absence of the PCA Supervisor, this position will seek direction and oversight from the Center Manager and Nursing Supervisor.

The position will be an IDT member and huddle with the PCA I, II, and assigned RN/LVN/HCC LVN using the PCA III communication tool to gather information to share at the IDT meetings.

This position will help support PACE participant’s engagement in satisfaction surveys, IDT discussions, physical and mental well-being and overall cost containment of participant’s supplies.

Identify and train on competency for PCA’s.

Assist PCA Supervisor in coordinating team assignments and workflow; disciplinary actions, and performance evaluations for PCA staff.

Responsibilities: 1.

Assist PCA I and PCA II to determine needs for participant care and assistance in activities of daily living (ADL) (IADLs), such as bathing, dressing, and toileting, eating and ambulating.2.

Collaborates with the assigned RN/LVN/HCC LVN to ensure all participants’ skilled needs are met and validated.3.

Assist in the provision of health education and counseling to participant when indicated by the plan of care and presenting in interdisciplinary team (IDT).4.

Provide assistance to the PCA I and II with cost containment in obtaining medical services from providers, such as incontinent supplies.5.

Assist in the development and implementation of the nursing component in the plan of care in conjunction with the interdisciplinary team (IDT).6.

Assist in the provision of supportive and restorative nursing as indicated by the treatment plan by engaging participants during exercise.7.

Notify direct Supervisor of any reported or observed change in participants’ health status to facilitate further assessment of their condition.8.

Assist and/or oversee the collection of diet cards for each participant/patient, meal distribution and arts/crafts distribution.9.

Provide oversight with participant’s personal care such as bathing, dressing, center laundry etc., as needed.10.

Ensure that PCA staff record arrival and departure times and all other required documentation for MSR and medical record-keeping on a timely basis.11.

Oversee and coordinate all PCA related services with direct Supervisor to ensure compliance with MD’s, PA’s, RN’s, therapist’s and nutritionist’s orders.12.

Mandatory attendance and active participation in the interdisciplinary team (IDT) meeting proceedings.13.

Assist PCA Supervisor in coordinating team assignments and work flow; includes training, disciplinary actions, and performance evaluations for PCA staff.14.

Perform other related duties as assigned.

Qualifications: 1.

Certified Nursing Aide education and training.

Nurse Assistant Certificate by the State of California, Department of Health and Human Services
– Aidand Technician Certification.2.

Current CPR/First Aid Certificate required.3.

High school diploma required or some college education/credits, preferred.4.

California Food Handler Certificate of Achievement upon employment preferred, must attain w/in 30 days of employment.5.

A minimum of 3 years’ experience working with physically or mentally impaired older persons strongly preferred.6.

Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred.7.

Experience working with the elderly patient population.
