SUMMARY OF RESPONSIBILITIESThe Outreach Coordinator II is responsible for coordinating the Street-based Outreach and Engagement System for individuals, family, and youth experiencing homelessness in Service Planning Area (SPA) 2.

The Outreach Coordinator will work closely with the Director of Engagement, Individuals to ensure seamless connections with community-based organizations and county entities are established to reduce street homelessness.

Primary responsibilities include, independently working within the Regional Coordination team in developing and implementing a cohesive system of response to individuals, family, and youth experiencing homelessness living on the streets, among diverse community providers and the appropriate Coordinated Entry System (CES) provider; executing a strategic response to incoming outreach requests from the County electronic outreach request and information system (LA-HOP – Los Angeles Homeless Outreach Portal); creating and implementing systems to track engagement responses among partner agencies; and coordinating outreach assignments with internal LAFH outreach teams and external Service Planning Area (SPA) 2 Outreach teams.

SPECIFIC DUTIESTrain & inform SPA 2 Outreach teams regarding new community resources and best practices within the SPA 2 Coordinated Entry SystemUtilize relationships of SPA 2 Outreach Leadership and leadership from partner agencies to assist SPA 2 outreach teams in following procedures for community resources to increase success of participant referral attainmentLiaise with LAHSA and the County electronic outreach request and information system (LA-HOP) to deploy SPA-specific outreach requestsProvide technical assistance to all providers utilizing LA-HOP.

Technical assistance includes one-on-one training for any staff member in SPA 2 who requires additional support to utilize the LA-HOP outreach deployment system effectivelyProvide support and onboarding opportunities regarding existing Outreach Coordination practices to all new LAFH staff and external outreach staff in SPA 2 to enhance the outreach response systemAssist in identifying & updating inventory/contact lists of all SPA 2 entities who are currently engaging individuals, family, and youth experiencing homelessness.

This includes providing excellent communication and response of Outreach Coordination updates and procedures to all entities via e-mail correspondenceCultivate relationships with supervisors from the different SPA 2 agencies, and communicate effectively all new outreach strategies and practices from the county strategyAssist in creating and implementing a strategic plan on outreach response efforts to maximize new and existing resourcesUtilize and analyze LA-HOP Data to inform necessary improvement in the outreach response system specific for SPA 2Facilitate meetings with existing SPA 2 Outreach partners related to LA-HOP data to implement necessary changes to the SPA 2 outreach response systemPrepare detailed tracking reports of outreach efforts and outcomes that result from coordinated outreach response, and present outcomes to internal and external staff involved in SPA 2 CESWork as a member of the CES Regional Coordination team to ensure the system is functioning effectivelyIdentify areas of improvement in the existing outreach response system and formulate effective solutions independently to address or remove barriers to streamlined responses.
