Kei-Ai Los Angeles Healthcare Center is currently seeking a Social Services Designee for it’s 300-bed skilled nursing facility which is located minutes from DTLA.

If interested and qualified, please submit your resume.Duties and Responsibilities:Provides customer service in an ethical, timely, accurate, efficient, and respectful mannerMaintains safe work environmentAssesses new residents, interviews family members of significant others and collaborates with the interdisciplinary team to develop the social services portion of the individualized resident care planCommunicates effectively with appropriate persons, including but not limited to, residents, families, volunteers, physicians, consultants, community resource centers, ancillary providers, support groups and the interdisciplinary team to identify and address resident needsAssists in coordinating transportation and medically related ancillary services as requestedProvides resource information and assists residents, families including, but not limited to, Social Security, Medicare and Medi-Cal programs, hospice, home-health, advance directives, resident rights, abuse, grievance, safeguarding valuables, psychotropic medications and counseling, medically-related ancillary services, community and legal resources and other health care facilitiesAssists residents, families and responsible parties to develop and implement advance directivesParticipates in discharge planning and the development and implementation of social care plans and resident assessmentsMaintains and promotes the confidentiality of all resident care informationCoordinates room changes, transfers and discharges as neededAddresses resident and family concerns in a timely and appropriate manner in accordance with the facility’s grievance policies and procedures*Qualifications:Candidate must possess a Bachelor’s degree or higher in Social Work or related human services field such as Psychology, Gerontology or Sociology.

Master’s level candidates preferredMinimum of 1 year functionally related work experience in a health care environment preferredMust have interest in working with elderly population in a nursing homeJapanese language proficiency preferredCompany Introduction:A highly sophisticated Skilled Nursing Facility in the heart of Los Angeles.
