Summary
We are looking for a compassionate, capable, and caring individual as a Full-Time Intake Coordinator to work out of our corporate office in Long Beach. The Intake Coordinator will be responsible for the coordination of home care services to prospective clients and referrals. You will evaluate clients referred to our organization, identifying insurance coverage, and coordinating referrals for home services. We are looking for someone to join our Private Pay Division and our Respite Division.
This is not a remote position.
Job Duties
* Handling all incoming referral calls and completing referral forms
* Communicates with and documents all communications and authorizations with staffing coordinators and other necessary departments
* Collaborates with discharging personnel, insurance, case managers to facilitate safe, early discharge
* Maintains liaison relationship with hospital and insurance personnel, families, referrals, providing information and education on our organization s services, coverage issues, and related areas of expertise
* Responsible of conversion of prospective clients and referral sources
* Responsible for transcribing detailed and accurate information in the computer system correctly to include but not limited to demographics, insurance information and prospective client information
* Communicates with client and family regarding arrangements for initiation of care/services
* Takes and prepares intake referrals, and prepares clinical notes
* Works closely with Staffing Coordinating department to ensure adequate care
* When requested, provides report to staffing coordinators or third party payers
* Facilitates ongoing interdisciplinary assessment of patient
* Perform other duties assigned by supervisor
Minimum Qualifications
* Must be Must possess excellent oral and written communication skills
* Must be friendly, professional, and cooperative with good aptitude of customer service and problem solving skills
* Ability to learn new software applications
* Ability to develop strong working relationships, possess team organization and community coordination skills
* Ability to work with minimal supervision, and utilize clear, concise, and positive communication skills
* Basic medical terminology
Job Requirements
* Must be 18 years of age or older
* U.S. work authorization
* High school diploma or equivalent
* Ability to pass a background check and reference check
* Must be bilingual in Spanish (preferred)
