$5,000 Sign On Bonus Responsibilities: 1.
Follow the interdisciplinary plan of care, be alert to the patient’s needs and concerns2.
Provide personal care, including bathing, dressing, grooming, hair, nail, skin and foot care and oral hygiene3.
Assist patients, according to the plan of care, a.

Turning ii.

With elimination, including; i.
Facilitation of bedpans, and urinals ii.
Assisting patient to the bathroom iii.
Routine Foley care iv.

Meals, when necessary4.
Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care5.
Observe and report changes in the patient’s condition and needs to the Team Director (TD) or Registered Nurse (RN)6.
May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping in the patient’s immediate area7.
Complete required documentation per policy8.
Participate in continuing education programs9.
Follow confidentiality regarding patients and families per policy10.
Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization11.
Attend patient care conferences as requested by the TD12.
Perform other related duties and special projects, as assigned13.
Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e.
RN, SW, Chaplain, MT, HA, etc.).

High School Diploma or equivalent, preferred2.
Certified by the state(s) as a nurse’s aide, having completed a course and certified by the state a.
In addition to CNA certification by the state: i.
CA: The Hospice Aide will be certified by the state as a Certified Home Health Aide (CHHA) ii.
GA: Will have completed and can provide validation of documentation of having completed a home health aide training and competency evaluation program conducted in a Medicare-certified home health agency; or will have successfully completed a personal care aide training program that is provided by the hospice under the direction of an RN iii.
MD: The Hospice Aide will also have passed a geriatric aide test, and be certified as a Certified Geriatric Nurse’s Aide (CGNA) iv.
NJ: The Hospice Aide must be certified by the state as a Certified Home Health Aide (CHHA)3.
Current and valid CPR certification, preferred a.
Direct Patient Care, required in Georgia & California4.
Listed on the state aide registry with a clear record able to deliver care to patients in their own homes, nursing facilities, or hospitals5.
Successfully complete the agency Home Health Aide Competency Exam with an 85% or better a.
CT: Minimum of six months experience in a supervised setting (preferably a healthcare setting)6.
Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance7.
Meet the regulations and requirements of the sate(s) in which the program provides service8.
A compassionate attitude toward the care of the sick; the ability to read, write and carry out directions maturity, and the ability to deal effectively with the demands of the job9.
