Help Me Grow Care Coordinator

AbilityPath is hiring a Coordinator for our Help Me Grow (HMG) program The Help Me Grow Coordinator will be the primary resource for families and staff for questions regarding child development.

HMG is a grant-funded system that offers families opportunities to ensure the optimal health of their children by supporting early identification of developmental concerns and providing referral(s) to link families to community resources based on their priorities and needs.About Us:We are a non-profit organization that provides a variety of programs and services for the lifespan of individuals with disabilities for the past 100 years Our mission is to empower people with special needs to achieve their full potential through innovative, inclusive programs and community partnerships.

from families and providers (e.g.

county clinics/private practice, mental health, early childhood programs, WIC, 4C’s, SMCOE, IHSD, public libraries, etc.)Follow HMG SMC Call Center protocols for answering calls/text/emails/web inquiries, triage, follow-up, mandatory reporting, and raise escalation of issues to supervisorsPromote and process ASQ online developmental screeningsProvide short-term care coordination to families with children between 0-5 yearsMake appropriate referrals for community and developmental servicesMake and receive warm referrals to the Family Resource Center team using Warm Referral ProtocolsEnter all family and provider data into data collection system in a timely manner according to the workflow protocol.

Ensure that data is maintained current at all timesRun regular reports through database systemRegularly identify families experiencing barriers within the following systems (GGRC, Spec Educ, HPSM & MH)Develop collaborative relationships with partners to coordinate and implement Tu y Yo parent/child programThrough outreach, develop collaborative relationships with community agencies, project lead agencies and partners to support the HMG SMC systemProactively research and participate in provider and family outreach events (e.g.

resource fairs, meetings, events, etc.) to promote the HMG system and Call Center.

Enter outreach data in data collection systemMaintain resource database with vetted resources (e.g.
