Contact us today to learn more Responsibilities: As a Field Nurse at Landmark Health, youll visit patients in their homes to address urgent medical needs and help them avoid unnecessary hospitalizations.
During every in-home patient visit, youll collaborate with the supervising Landmark provider by phone or video to determine the appropriate evaluation and care.
Youll help patients by evaluating their health, providing treatments, educating patients on their conditions, documenting the interaction, and helping to coordinate follow-up care.
Qualifications: Requires a current LPN or LVN license and a high school diploma or equivalent Ability to work a variety of shifts.
Access to reliable transportation to conduct patient visits required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy.
Urgent care, emergency room, or community home health experience preferred.
Comfortable working with electronic medical records and mobile technology.
Solution-oriented mindset and work approach.
Charisma and a natural ability to connect with others.
Excellent communication and interpersonal skills, with a strong emphasis on empathy and building rapport with patients.
1 years of experience evaluating, assessing and providing patient care.In order to work with potentially vulnerable patient populations and to keep employees healthy while caring for patients, all patient-facing employees of Landmark must provide evidence of the following required immunizations/vaccinations as a condition of employment: TB, MMR, Tdap, and Influenza.
