The Child and Family Guidance Center provides comprehensive and collaborative outpatient mental health services to children, adolescents, young adults, and their families throughout the San Fernando and Antelope Valleys.

The Position:

We are seeking a full-time Case Manager – Transitional Youth Specialist to be responsible for providing case management services to the aforementioned population in a mental health setting in the San Fernando Valley.

Job Requirements:

  • Essential Job Functions:

    • Meets with referred youth and/or parents to conduct an initial transitional youth assessment and develops a case management treatment plan to address related concerns.
    • Communicates and coordinates case management services with the referring therapist and/or psychiatrist.
    • Researches available transitional, educational, and associated resources that will meet the client’s needs and links clients to appropriate resources.
    • Establishes and maintains professional relationships with community agencies, schools, and higher education programs through phone contacts, meetings and/or visits with clients.
    • Attends IEP meetings and serves as an advocate for the client and parent.
    • Teaches job preparation and independent living skills in both an individual and group setting.

    Skills You Need:

    • Bilingual in Spanish and English preferred.
    • Two years minimum experience working with teens and/or young adults in a mental health, school or related setting.
    • Ability to provide services in a warm and supportive manner with children who have behavioral, academic and social/emotional difficulties.
    • Ability to work with an interdisciplinary team, form and maintain professional relationships, and accept and utilize supervision from clinical staff.
    • Must have strong organizational, time management and writing skills.
    • Must be a self-starter and be able to work independently

    Hours:

    • Monday through Friday (40 hours)
    • Hours: 9:00 a.m. – 5:30 p.m. (with some evening hours required)
    • Meal Period: 30 minutes

    Education Required:

    • BA/BS degree
    • Minimum of two years mental health, school or related experience preferably with teens or young adults.
