CRCF Administrator

We are looking to hire a Resident Services Coordinator who can be responsible for providing a supportive, service-oriented environment to senior and/or physically challenged residents living at TELACU Residential Management (TRM) housing communities. The Resident Services Coordinator (RSC) will be assigned specific housing communities to work in conjunction with on-site staff.

Essential Job Duties

  • Provide case management to assess residents for assistance and self-disclosed care needs; assist residents with ongoing delivery and receipt of services in response to changing needs and/or circumstances.
  • Monitors the on-going provision of services, establishes links and builds relationships with all agencies and service providers in the community to ensure that the necessary services are available and delivered in a collaborative and efficient manner to benefit residents.
  • Make appropriate referrals to service providers on behalf of residents based upon needs and requests.
  • Establishes and develops ongoing education presentations, wellness programs, obligations of tenancy presentations and on-site approved services for residents.
  • Serves as resident advocate and teaches residents how to advocate for themselves.
  • Serve as a liaison to the external community, helping to educate community organizations about the resident services role/program, and develop partnerships to support the residents to age in place.
  • In collaboration with local hospitals/SNF’s/Assisted Living locations and their personnel, review discharge plans to help and ensure timely follow-up of needed supports and coordination of care for residents discharged from the emergency department, hospital, skilled nursing facility, or home health care.
  • Develop informal and formal support networks with other residents, family and friends in an effort to support residents to age in place.
  • Interpreting / Translating: Provide language services to Residents with limited English proficiency. Facilitate effective communication between two parties that do not speak a similar language by converting spoken, signed, or written language to another.

Other Qualifications:

  • Engage and motivate residents, including socially isolated individuals, to enroll in the program and be active participants in improving/maintaining their health and well-being.
  • Provides necessary support to residents related but not limited to eviction, crisis prevention, and conflict resolution
  • Manage onsite transportation program by creating monthly trips to facilitate necessary services within the community
  • Creates, organizes and manages PUPM funds and programs at sites with available funding
  • Establish and maintain communication channels with community agencies and property management staff to appropriately share information concerning residents and identify ways in which resident needs can be efficiently addressed.

Knowledge and Skills:

  • Basic computer proficiency, including ability to maintain data in an electronic database, communicate via e-mail, use internet, and operate word processing software to create program materials.
  • Must have foundational computer skills including: Microsoft Office Suite (Outlook email and calendaring) and the Internet
  • AASC Online and HUD provided Software; not required at the time of employment

Experience and Education:

  • Bachelor’s degree in Social Work, Gerontology, Psychology or Counseling or equivalent combination of education and experience
  • Ability to advocate, organize and problem-solve
  • Ability to translate verbal and written communication into easily understandable lay language and to communicate at appropriate literacy and cognitive levels
  • Understanding of and sensitivity to the needs of low-income older adults and persons with disabilities
  • Ability to establish positive and collaborative relationships with residents, staff and community-based providers
  • Experience working with older adults and special needs
  • Must be bilingual English/Spanish

Physical Requirements:

Constantly (6-8+ hours per day) – Sitting, Using hands to finger, handle or feel (computer operation)

Occasionally (Up to 3 hours per day) – Standing, Walking, Stooping/Bending, Reaching with hands and arms, Lifting of up to 20 lbs

Work Environment & Exposures:

Position may require occasional exposure to fumes or airborne particles, vibration and loud noise levels. Will be exposed to varying weather conditions, soil, pollen, grass and plant materials and fertilizer while working; will be exposed to cleaning solutions of home strength; may be exposed to industrial strength solutions for pest control, paints or solvents. Hazards: Occasionally exposed to biohazards such as blood borne pathogens, sewage, and building waste.

COVID-19 Vaccination:

Must be fully COVID-19 vaccinated by the first day of work. A copy of the CDC COVID-19 Vaccination Record Card must be submitted to Human Resources.
