The Community Engagement Navigator is responsible for engaging with Anthem members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between Anthem and referral sources.

Primary duties may include, but are not limited to:

Engage with members who have been identified as possessing significant SDOH barriers.

Work with the member to establish goals and next steps for addressing SDOH.

Support member in selecting provider or programs most appropriate for supporting their needs.

Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.

Act as a liaison between referral sources and the health plan.

Qualifications

Minimum Requirements:

BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.

Local Travel is required.

Preferred Requirements:

Experience with behavioral health or county mental health department.

Knowledge of Microsoft Office (Word, PowerPoint, Excel, Outlook, Etc.)

Knowledge of Vocational Rehabilitation preferred.

Bilingual preferred

Los Angeles, CA area preferred.
