The CEDIS also cultivates, enhances and maintains a cooperative and effective relationship with clinical staff. ESSENTIAL DUTIES AND RESPONSIBILITIES: In collaboration with Sexual Health Program Manager and other CEDIS staff, serve clients with STDs including locating and following-up with sex partners for appropriate and treatment. Interview clients to gather sex partner(s) information in an attempt to identify the source of infections and to prevent further disease transmission in strict adherence with confidentiality laws. Provide client-centered counseling and active referral(s) to relevant social and health services for clients diagnosed with an STD. r newly diagnosed HIV individuals or those who need to be re-engaged in medical care. Conduct field investigations, independently (site visits to clients homes, neighborhoods, shelters, etc.) to locate/notify clients of test results or exposure, interview clients, and refer individuals for necessary medical evaluation and treatment. Accurately enter client information into Case Watch or other database as required by DHSP. Attend DHSP Partner Services unit meetings and trainings as needed. Attends department meetings and trainings. Contributes to data collection to measure quality and performance. Sets yearly program objectives/goals. Submits monthly and year end reports. Represents APLA Health & Wellness in a professional manner at a variety of roundtables, community forums, town hall meetings and other venues where the issues regarding the provision of HIV and/or STD services are discussed. Ensures contractual compliance. Prepares quantitative and qualitative reports. OTHER DUTIES MAY BE ASSIGNED TO MEET BUSINESS NEEDS. Job Requirements: raining and Experience: Bachelors degree and 2 years of experience working in public health, nursing, social work, health sciences, communications or community organizing OR High School diploma and 4 years of experience working in public health, nursing, social work, health sciences, communications or community organizing. Experience with health education/health promotion and program development. Familiarity with the harm reduction model. Experience in a nonprofit environment preferred. Strong oral and written skills required.Strong analytical skills are needed to solve daily work issues and/or work flow challenges. Must pass LA County’s Division of HIV &STD Programs CEDIS training during first year of employment. Knowledge of: HIV infection; STD infection/treatment; HIV prevention; psychosocial issues as it relates to HIV and STD transmission/acquisition, special issues relevant to individuals who are at risk for HIV/STD and/or living with HIV;Health education and behavior modification theory; human sexuality, substance abuse, behavior change principles and strategies; effective communication skills; group dynamics; data management; general computer knowledge; community resources;Health and social concerns of diverse populations such as gay and non-gay identified men who have sex with men and other genders, individuals who share injection paraphernalia, heterosexuals, women at sexual risk and transgender individuals. Ability to: Communicate clearly, work well in a multidisciplinary team and be able to work with a wide variety of people in a range of community and clinical settings. Maintain a high degree of professionalism and the ability to handle confidential information at all times. Provide services in a non-judgmental fashion and work effectively with diverse populations, Work independently with minimal direction. Work in a fast pace environment.Meet multiple deadlines in a rapidly changing environment. Maintain records and follow clinical guidelines/protocols. Work efficiently and complete tasks with a high degree of accuracy. Work flexible hours. Travel (local) to conduct field investigation; attend meetings at health centers and other relevant meetings. WORKING CONDITIONS/PHYSICAL REQUIREMENTS: This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a personal computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper. SPECIAL REQUIREMENTS: Must possess a valid California drivers license; proof of auto liability insurance; and have the use of a personal vehicle for work related purposes.
