Candidate must reside in the Los Angeles area for full consideration for this remote roleThe Patient Navigator is responsible for providing direct patient assistance to help individuals obtain necessary breast services and breast cancer support throughout the continuum of care. Navigator will provide direct assistance to individuals and communities that results in (1) access and adherence to screening, diagnosis, and/or treatment services, (2) culturally competent breast health and breast cancer education (3) culturally competent navigation, including patient-provider communication, managing cancer-related distress, focus on quality of life and financial barriers. and (4) reduction of financial barriers for uninsured and underinsured individuals. This position requires an ability to build strong relationships with community organizations, volunteers, colleagues, and the health care community. Primary Responsibilities: Assist clients with various breast health and breast cancer-related needs to eliminate barriers to care. Facilitate appointments and connections to local resources and provide follow-up to all clients to provide wrap-around care.Conduct outreach to create, establish and maintain positive working relationships with key community partners, including health care providers (particularly nurse navigators), social workers, and social service organizations.Provide risk-assessment information and referrals for genetic counseling and screening, as appropriate. Work with the state Breast and Cervical Cancer Program to recruit and enroll eligible clients into the programContinuously identify and maintain current resources and eligibility requirements to monitor changes and program availability.Track program data, case notes, and partnership meetings and maintain client and program files.Document all interactions in navigation tracking tool for evaluation and reporting purposes.Serve as an essential link between clients and community resources.Identify appropriate educational material and resources to follow-up with clients All other duties as assigned. Position Qualifications:The ideal candidate will have: Experience as a breast cancer patient navigator. Experience serving the Black community. Ability to guide individuals in high-stress situations to the care they need with a calm, culturally competent, non-judgmental demeanor both over the phone and in-person. Comfortable discussing breast health, cancer and related topics. Strong problem-solving and organization skills with the ability to prioritize and make decisions to deliver multiple objectives simultaneously. Demonstrated computer proficiency in MS Office Products and ability to learn new software. Work Experience: 2 – 5 yearsEducation: Associate Degree: (Medical and health professions, psychology, human behavior, social work)Preferred experience includes: Prefer candidates with proven experience as a community health worker or patient navigator working with vulnerable populations. In addition to the qualifications above the successful candidate should have: Prefer Licensed Community Health Worker or Certified Patient Navigator.
