Aston Carter: Care Navigator, Medication Advisor

MUST LIVE IN CALIFORNIA MUST BE BILINGUAL IN EITHER: SPANISH, OR MADARIN, OR KOREANDescription:Care Navigator Continuous RZHigh volume outbound calls, making about 400 calls per monthWork in caseloads approximately 1500 cases yearlyConducting medication adherence assessments over the phoneThey will be doing motivational interviewing they are given a script with open ended questionsActive listening, asking probing questions, critical thinkingThey will recommend solutionsCall pharmacies and providers if needed for follow-upConducts the Medication Adherence assessment and needed follow up on members identified as high risk through the Pharmacy Department.

Enhance member satisfaction and access to quality care by identifying member needs, medication adherence gaps, providing coaching to members on how to be more adherent, facilitating access to appropriate and community programs, and proactively assisting members with obtaining necessary healthcare services.

Act as a liaison among members, Provider Organizations and community-based organizations.

You willConduct telephonic outreach to members with identified medication adherence issues; conduct assessment and all necessary follow up assessments to support member to become adherent with medication use.Document assessment information in the care management system during the phone call with the member and/or caregiver and documents all corresponding information or notes per department policy.Make outbound calls to members based on identified health interventions or quality initiatives.

Encourages and provides assistance to members who may encounter obstacles to seeking care.Utilize department desktop procedures, workflows, job aids and training material.

Identifies barriers to work processes and brings to the attention of the supervisor/manager.Maintain the members right to privacy and protects operations by keeping information confidentialAdhere to all quality, compliance and regulatory standards to achieve Health Care Services and outcomes.Actively support the achievement of Vision and Goals.Contribute to team effort by accomplishing related results as needed.Skills:Pharmacy Tech, Bilingual Spanish, Health, AssessmentTop Skills Details:Pharmacy Tech, Bilingual Spanish, Health, AssessmentAdditional Skills & Qualifications:Must have a high school diploma or G.E.D.

2-3 years of medical assessment experience; or related healthcare experience.

Demonstrated knowledge of assessment, health and functional problems of older adults and managed care.
