The Service Coordinator provides customer service to participants which includes, but is not limited to, completing intake meetings, ongoing monitoring of services, and maintaining communication with the client, their designated representative(s), and county case manager.

The Coordinator serves as a resource to other Accra employees about Basic 245D Services.

The Coordinator is expected to stay abreast of current policies and changes to rules and regulations related to Basic 245D services, and to participate in committees and meetings to enhance their knowledge.

Requirements: 4 year degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or2-year degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; ora diploma ina field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; ora minimum of50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed above.

Essential Functions: Meet with clients and representatives to complete intake process, explain employment process, and initiate services.Educate clients on role and responsibilities of client and/or Responsible Party.Monitor and document service delivery on an on-going basisProvide technical assistance and information customer service support as neededDocument and report non-compliance events to appropriate party.Participate in quality monitoring activities and provide technical support to assure compliance with regulations and policies.Maintain knowledge and understanding of DHS HCBS waiver regulations, 245D Statutes, and best practices regarding participant-directed servicesMaintain knowledge of Accra’s policies and procedures.Participate in agency and community outreach and trainings at the request of the supervisor About Accra: Accra is a homecare organization dedicated to individualized person-centered care delivered at home.
