Promote the maintenance of optimum health and reduction of potential injury.
Duties:1.
Monitor the health and well being of up to 30 participants according to OPWDD regulationsAttend relocation/pre-admission, annual meetings and review medical records including current medications and treatmentsObtain medical services for all participants and assist with coordinating services with medical personnel and other AgenciesComplete nursing assessments annually and semi-annuallyEnsure maintenance of orderly medical records for each individualComplete pharmacy reviews as requiredSchedule, accompany, advocate, and /or transport participants to medical appointments as neededReview all Physician Health Report Forms after physician (or other provider) visits and assure follow up on next appointment and provider’s recommendations/prescriptionsDevelop and monitor Plan of Nursing Services (PONS)Complete required Nurse documentation as required by OPWDD and Agency rules and regulations 2.
Staff TrainingTeach AMAP course as needed to ensure certifications and provide annual re-certification in compliance with OPWDD regulationsProvide on-going RN trainings regarding infection control, bloodborne pathogens, signs and symptoms of illness, including program participant’s medical needs and medications, first-aid and emergency medical proceduresObserve and review staff competency in medication administration during regular site visits and make recommendations as needed 3.
ProfessionalAttend meetings in community as an Agency Registered Nurse representativeAttend scheduled medical review meetings with Program Managers and Program CoordinatorParticipate in Quality Assurance reviews and Total Quality Management Teams to identify and implement systematic improvementsAssist with follow-up and implementation of recommendations from internal and DQI auditsParticipate in interdisciplinary meetings as needed 4.
TeamworkAttend and participate in department, team, and staff meetings as requiredAssist other AccessCNY Registered Nurses as neededProvide orientation and ongoing training and support to team membersComplete site observations and report concerns to Program CoordinatorBack-up for Human Resources Registered Nurse for TB screening as needed5.
Manage on call responsibilities 6.
Perform other duties as assigned Scope of responsibility:RN works independently and is able to use personal and professional discretion in implementing new approaches and/or problem solving.
With minimal supervision, prioritize outcomes, provide training and interact with other medical providers in the community.
Contact with the public is extensive.
Education and Experience:Registered Nurse, licensed in New York State.
Two years experience with people with disabilities preferred.
Conditions of Employment:Must have a valid NYS driver license in accordance with Agency Vehicle and Driving Policy and transportation necessary to meet program needs.
All Registered Nurses must be available for emergencies.
Schedule flexibility to accommodate participant needs.
Must be able to lift 75 pounds.
Must participate and meet established requirements in a database checks through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Developmental Disabilities, Chapter 575 of the Laws of 2004 and the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
