We are hiring for an Outreach Coordinator on our team in our Fullerton office.

We are working remote right now but plan on transitioning back to the office in a couple of months.Responsibilities:RESPONSIBILITIES:Coordinate services for new consumers referred by the Regional CentersConduct intakes over the phone & create individual service plans based on care and schedule needsServe as the first line of communication for consumers while responding to all questions and concerns in a timely mannerWork closely with Operations to communicate consumers’ needs and prepare for new agency consumer requests, respectivelyFollow up with families awaiting services and keep Regional Center staff updated and informedAssist in fostering relationships with current or potential referral sourcesQualifications:QUALIFICATIONS:1-2 years of customer service experienceBachelor’s degree, preferredSpanish speaking requiredEnsures that referrals are addressed and documented within the proper timelines that are outlined in the office’s Referral Response and Tracking PolicyPositive “Can Do” attitudeProblem solving skillsStrong interpersonal and communication skillsEnergetic self-starterAble to exercise discretion with confidential informationBENEFITS & PERKS:COVID-19 RESPONSE:As an essential service, 24 Hour Home Care is committed to being part of the solution continuing our business and looking for innovative ways to support our clients, partners, and communities as we weather the storm together.
