Social Worker (LCSW) Full Time, Day Shift – Palliative Care

Now is your chance to apply your passion to our mission.We’re looking for someone to join our team as a Palliative Care, Licensed Clinical Social Worker (LCSW) who will be dedicated to serving the inpatient palliative care patients. This position will require a highly driven, passionate, self-motivated, and thorough person that is bilingual Spanish/English who will serve to coordinate the inpatient palliative care program, develop and report quality metrics, and facilitate implementation of evidence-based best practices to promote excellent outcomes for patients and families.Essential Functions:*

  • Development and implementation of evidenced-based Palliative Care program* Ability to measure and report quality metrics related to palliative care team performance* Collaboration and communication with multiple disciplines* Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care* Assists patient/family/caregiver in determining appropriate level of care* Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient.* Develops social work Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics. Documents relevant data accurately and in compliance with laws and regulations* Actively participates in training and/or cross-training of duties* Provide a high standard of holistic care encompassing physical, psychological, spiritual and social care to patients and families* Offers specialized knowledge and skill in the care of terminally ill patients and their families* Disseminates specialist knowledge to hospital associates and members of the primary health care teams* Performs other job-related duties as assignedYou’ll be successful with the following qualifications:Education:* Master’s Degree in social work: RequiredWork Experience:* One year experience in an acute or long term medical setting: Required* Two years experience in an acute or long term medical setting: PreferredRequired Licenses/Certifications:* Licensed Clinical Social Worker (LCSW): Required
