Our Supportive and Health Services Department consists of a team of passionate and dedicated professionals who specialize in providing high level support services to current and formerly homeless individuals, physically and/or mentally disabled individuals, vulnerable seniors and veterans. About the Position The Licensed Care Social Worker (LCSW) position will support newly housed, formerly homeless individuals and families. Responsible for providing initial evaluations and coordinating client care with Intensive Case Management Team. Will be responsible for providing Intensive Case Management Services to a small client case load. Core Duties include the following, but not limited to: Promote, coordinate and tailor clinical support services to clients Intensive Case Managers to meet identified needs. Assist Intensive Case Managers individually or in small groups regarding plans for meeting client mental health needs; aid in mobilizing client capacities and environmental resources to improve social and economic functioning. Lead or assist with Crisis Management in clinical support to Intensive Case Managers Meet with individual, high risk clients to evaluate mental health status and potential need for higher level needs and provide therapeutic services to clients as needed to assist in maintaining housing May maintain a case load of 3 to 7 higher risk clients in need of clinical support to maintain housing Review service plans with Intensive Case Managers and suggest client goals to improve and support mental health needs Monitors and coordinates the delivery of mental health services for clients, including assisting Intensive Case Managers in linking with other treatment providers Provide and communicate recommendations to Intensive Case Mangers for interventions Work collaboratively with a wide range of clinical and non-clinical disciplines to assess client needs and work with Intensive Case Manager to design effective care planning or service goals or medical treatment plans regarding utilization of additional resources Design and facilitate specialized trainings to assist Intensive Case Managers in growing case management skills with a high-risk population with mental health needs May need to work some evenings and weekends in order to attend events or address situations of crisis. Compensation & Benefits This is a 30 hours per week, non-exempt position, offering a competitive salary range of $40.00-45.00 per hour, depending on skills and experience.
