THE POSITION:

Under the direct supervision of the Program Manager, the MCC Case Worker will work with patients to address barriers to linkage and retention in care. The Medical Care Coordination (MCC) model uses a multi-disciplinary team of a Patient Care Manager, a Medical Care Manager, and a Case Worker, who work together to facilitate behavioral interventions and coordinate support services to promote improved health outcomes for PLWHA. MCC team members are co-located at the patient’s medical home and deliver patient-centered activities that focus on addressing health status, engagement and retention in care, adherence to HIV medications, and HIV risk reduction.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: (This list may not include all the duties assigned.)

  • Identifies and enrolls persons with HIV/AIDS into a case management program providing services within the MCC programs.
  • Works as part of an interdisciplinary team to formulate care plans based on assessment and provision of care priorities.
  • Coordinate access to services including nutrition services, legal services, in-home support, attendant care, and skilled nursing.
  • Actively participates in Case Conferences to determine appropriate care plans for patients.
  • Delivers brief interventions focused on patient education, treatment adherence, managing side effects, medical nutrition therapy, coinfections, and preventative care.
  • Develops informational and educational materials and various forms for programs.
  • Cultivates and develops community social services linkages for patients and families with HIV/AIDS.
  • Acts as the liaison between HIV Counseling and Testing sites and the medical clinic to ensure that new patients are enrolled in medical care seamlessly and in a timely fashion.
  • Attends staff meetings, programmatic training/meetings, various community meetings, task forces, and local consortiums.
  • Monitors and evaluates clients’ progress on regular basis.
  • Maintains security and confidentiality of client records.
  • Maintains complete, accurate, and timely documentation.
  • Keeps abreast of the latest medical information regarding HIV/AIDS and services for people affected by HIV/AIDS.
  • Educates clients regarding information and resources relevant to clients’ needs.
  • Other duties may be assigned, as required.

QUALIFICATIONS:

  • Either a bachelor’s degree in Nursing (BSN), Social Work, Counseling, Psychology Human Services, OR a license as a vocational nurse (LVN).
  • Minimum of 2 years of case management experience preferred or equivalent work experience within a related field of Health and Human Services.
  • Knowledge of community resources for public assistance.
  • Must be certified through the Los Angeles County HIV Case Management Certification process.
  • Excellent communication and organizational skills; strong written and interpersonal skills.
  • Proficient in Microsoft Office (Word, Excel, Access, PowerPoint).
  • Bilingual/Spanish preferred.

COMPANY REQUIREMENTS:

  • Must be able to pass a pre-employment drug test and background check to include a 7-year criminal, 10-year SSN & employer history reference check.
  • Excellent interpersonal skills.
  • Attention to detail.
  • Must be able to work flexible schedules.
  • Must have skills in research and development of operating policies and procedures.
  • Must take yearly flu shots or wear flu masks during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.

LANGUAGE SKILLS:

Must be able to read, write and speak the English language fluently. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must have the ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public in person.

MATHEMATICAL SKILLS:

Ability to calculate figures and amounts such as dollar totals, monetary conversions, discounts, interest, fractions, decimals, percentages, area, and volume.

REASONING ABILITY:

Ability to solve technical and practical problems and deal with a variety of intangibles in situations where only limited communication, support, information, and access exist. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

COMPUTER SKILLS:

Must have good computer skills and be proficient in the use of systems to complete correspondence, documents, spreadsheets, and presentations, such as Microsoft Outlook and Office; Word, Excel and PowerPoint, and Adobe Acrobat.
