SoCal Club Case Worker

THE POSITION:
Under the direct supervision of the SoCal Club Program Manager, the Case Worker will work SoCal Club members and MHF patients to address social determinants of health that serve as barriers to access to and engagement in care. The Case Worker will work with a multi-disciplinary team that includes a Behavioral Health Specialist, a Benefits Specialty, and other MHF staff to assess the needs of SoCal Club members and develop and implement a service plan to help them access needed services that promote improved health outcomes for YMSM of color.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: (This list may not include all the duties assigned.)
- Identifies and enrolls persons at risk for HIV and STIs into case management services who have needs accessing support services such as benefits specialty, behavioral health, medical care, vocational and housing, among others.
- Works as part of an interdisciplinary team to formulate care plans based on assessment and provision of care priorities.
- Actively participates in Case Conferences to determine appropriate care plans for members enrolled in case of management services at the SoCal Club.
- Develops informational and educational materials and various forms for case management participants.
- Cultivates and develops community social services linkages for SoCal Club members.
- Acts as the liaison between the SoCal Club and MHF South LA s medical clinic to ensure that new members are enrolled in medical care seamlessly and in a timely fashion.
- Attends staff meetings, programmatic training/meetings, various community meetings, task forces, and local consortiums.
- Monitors and evaluates client s progress on regular basis.
- Maintains security and confidentiality of clients records.
- Maintains complete, accurate, and timely documentation.
- Educates clients regarding information and resources relevant to clients needs.
- Other duties may be assigned, as required.
QUALIFICATIONS:
- Either a bachelor s degree in Social Work, Counseling, Psychology Human Services.
- Minimum of 2 years of case management experience preferred or equivalent work experience within a related field of Health and Human Services.
- Absent a bachelor s degree, a minimum of 4 years of case management experience is required.
- Knowledge of community resources for public assistance.
- Excellent communication and organizational skills; strong written and interpersonal skills.
- Proficient in Microsoft Office (Word, Excel, Access, PowerPoint).
- Bilingual/Spanish preferred.

COMPANY REQUIREMENTS:
- Must be able to pass a pre-employment drug test and background check to include a 7-year criminal, 10-year SSN & employer history reference check.

- Must have skills in research and development of operating policies and procedures.
- Must take yearly flu shots or wear flu masks during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
LANGUAGE SKILLS:
Must be able to read, write and speak the English language fluently. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must have the ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public in person.
MATHEMATICAL SKILLS:
Ability to calculate figures and amounts such as dollar totals, monetary conversions, discounts, interest, fractions, decimals, percentages, area, and volume.
REASONING ABILITY:
Ability to solve technical and practical problems and deal with a variety of intangibles in situations where only limited communication, support, information, and access exist. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
COMPUTER SKILLS:
Must have good computer skills and be proficient in the use of systems to complete correspondence, documents, spreadsheets, and presentations, such as Microsoft Outlook and Office; Word, Excel and PowerPoint, and Adobe Acrobat.
