In making your choice about where you will practice your art, consider the following:Our facilities are Intermediate Care Facilities (not SNFs) for people with disabilities, with both Habilitative (ICF/DD-H) and Nursing (ICF/DD-N) homesCaregiver to people supported ratio does not exceed 1:5 and is sometimes as high as 1:1Nursing caseload is 15-30Our home sizes are small (6-bed and 15-bed)At our long-term care homes, you have the opportunity to build relationships with the people we support and a team of staff committed to serviceComplex case management will keep you engaged and learningHigh-acuity care is limited or prohibited by facility regulationsOur Immediate Care Facilities provide wound care per licensing requirements ranging from simple wounds only to a maximum of stage I pressure ulcersHospice care is very limited; when provided, it is done collaboratively with professional hospice care providers who come to our homesRN Essential Responsibilities:Promotes a friendly, comfortable, and safe environmentRespects and promotes the right of individuals Provide direct RN assessment and care management of each supported person with a minimum of 1-1.5 hours per week at multiple locations*Quarterly nursing examReview and implement feeding plansProvide complex care management and vital assessmentsDevelop health care planSupervise LVN clinical responsibilitiesAccompany persons supported to medical appointmentsEnsure doctors’ orders are carried outPerforms other duties as assigned to meet department and/or business needsCase Management RN Responsibilities Include:Complete required nursing reportsDetermine LVN assignments and schedulesParticipate in IDT (Inter-Disciplinary Team) meetingsWork with Pharmacy Consultant for Pharmacy AuditsAudit MAR (Medication Administration Record) bi-monthlyAssist monthly in-service trainingOversee and support monthly cycle medicationsSchedule doctors appointmentsEnsure specialist referrals and appointments are properly scheduledPrepare persons supported charts for doctor roundsReceive and record doctors’ ordersAssist physician on roundsRN Qualifications: Graduate of a Board of Registered Nursing approved school; requiredValid/current California RN License; requiredMinimum of one year of full-time clinical experience as an RN; preferred Supervisory and management skills; preferred Excellent oral and written skillsStrong critical thinking skillsAbility to maintain confidentialityCommunicate and work professionally and cooperatively with othersExperience working with adults with disabilities and/or developmental disabilities; preferredSuccessfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United StatesAbility to run, push, pull, bend, squat, kneel, and lift up to fifty (50) poundsValid and current proof of legal right to drive in California; preferred; preferred *Sign on Bonus will be paid in two payments.
