Daily Responsibilities of a Telephonic Nurse Case Manager:
Responsible for creating and updating a plan of care and with the active involvement of the patient and their health care team including short- and long-term goals with timelines and priorities for achievement.
Requirements for a Telephonic Nurse Case Manager:
- Must have a clear & active RN CA State license
- 3+ years with Telephonic Case Management experience form Ambulatory setting
- 2+ years of Adult Managed Care experience: MediCal, Medicare, and Commercial
- Experience with creating care plans / Case summaries
Hours for this Telephonic Nurse Case Manager Position:
Monday-Friday, 9-530pm PST
