Staff Nurse II (Per-diem)

Overview: The GeroPsych Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at AHMC Seton Medical Center and Nurse Practice Act.

Responsibilities: Collects, reviews, and evaluates clinical data regarding assigned patients current medical and psychiatric signs , diagnoses and history.

Completes all admission assessments in time allotted and initiates protocols as per policy.

Assesses per unit policy both medical and psychiatric presentation inclusive of mental status and safety & security assessments of danger to self and/or others, and legal status assessments.

Assess and reassesses high risks patients, ensure that safety protocol in place and notify the whole team of the plan of care.

Completes skin and fall assessments and initiate protocols as per policy.

Analyzes data from the assessment to formulate and prioritize the plan of care.

Create care plan timely per Seton policy.

Interprets the geriatric and psychiatric condition and treatment needs of the patients and educate family members and non-medical staff members.

Provides and/or delegate patient care according to the plan of care and/or department protocol and procedures.

Prepares, administers, and monitors medication administration and records medications accurately.

Communicates pertinent information to other medical professionals on the interdisciplinary team including but not limited to physicians, therapists, nutritional services, pharmacists, case managers, and social service staff.

Reassesses and evaluates outcome of care provided and revises the plan of care as necessary.

Uses critical thinking skills when making care judgement, dialoging with patient/families and/or prioritizing patient care.

Promotes and maintains harmonious and positive relationships within the department and with other departments Presents and coordinates change of shift reports with proper hand-off procedures.

Ensures that pertinent information is passed on to the next care provider.

Provides education to patients and significant others during promotion, restoration and maintenance of health.

Facilitates therapeutic groups within scope of practice.

Acts as a role model in maintaining a hopeful and positive attitude in problem solving and coping with the disabilities associated with mental illness.

Conducts initial and ongoing assessments by appropriately reviewing, collecting and evaluating relevant clinical data for assigned patients.

Uses patient assessment and clinical data to formulate patient plan of care.Delegates and supervises patient care including patient assignments, assuming charge responsibilities and daily staffing needs.

Actively participates in multidisciplinary team and treatment planning meetings including group therapy.

Helps process intakes/admissions in timely manner when needed.

Establishes and maintains a therapeutic relationship with geropsychiatric patients.Timely completes the required Setons compliance policy such as Health Stream, RQI, employee health annual exam etc.

Demonstrates superb guest relation skills when caring for patients, their family members, and others.

Proficiently uses therapeutic communication technique and treats everyone with dignity and respect.

Participates in departments Performance Improvement Projects.

Responds to emergent situations and initiates appropriate actions efficiently.

Complies with the hospital and departments Safety policies and procedures and unit protocol, including standard precautions.

Assures equipment malfunctions are reported to the appropriate department i.e.

Engineering dept., IT, EVS and Biomed, etc.

Accurately documents in the medical record in accordance with the established policies and procedures.

Performs work in a timely, productive, and professional manner.

Constructively receives feedback and direction from co-worker or supervisor.

Proactively problem solves in real time, initiate efforts to mitigate future occurrence and reports to unit manager, nursing supervisor or administrator.

Maintains hospital and patient confidential information.

Assures that medical orders are transcribed and processed accurately.

Reports and communicates patient’s changes in condition, progress, and accurately documents in the medical record.

Assists with orienting all levels of new staff for the unit, including but not limited to serving as preceptor.

Follows the chain of escalation, brings issues forward to improve safety and quality for patients and staff.Assists and supervise C.N.A.s to maintain patients safety and provide quality care within his/her Scope of Practice.

Qualifications: EDUCATION: Graduate of an accredited School of Nursing requiredBSN or BA degree is preferredEXPERIENCE:Two years of clinical experience preferredTraining and experience in psychiatric nursing preferredInpatient psychiatric nursing is preferredCERTIFICATIONS/LICENSURECurrent California RN licenses requiredCurrent American Heart Association BLS certification requiredCrisis Prevention Intervention verification required within the first 3 months of hireOTHER SKILLS, ABILITIES & KNOWLEDGE:Ability to work effectively as a member of a multidisciplinary teamA thorough understanding of client rights, an understanding of the commitment process and knowledge of basic medical and psychiatric terminologyFamiliarity with behavioral health resources in the community is desired Hospital Description: AHMC
