Our available roles are Full Time as well as other opportunities that are Part Time or as a Per Diem Registered Nurse. As a Registered Nurse, you will be responsible for the completion of assigned visits, assessments, admissions, educating, instructing, and supporting the patient, family, and caregiver needs. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met. Provide timely documentation of patient services based on plan of care. Identify other physical, psychological, social and spiritual needs for assigned patients Requirements: Current RN license in California. Minimum one year experience in hospice, home health, acute care, or oncology setting. Demonstrated understanding of Hospice philosophy. Excellent written and verbal communication skills. Valid driver’s license, auto insurance, and reliable vehicle. Must be able and willing to travel locally within assigned territory. FLEXIBLE SCHEDULE | BILINGUAL IS A PLUS Licensed Vocational Nurse (LVN) HOSPICE FIELD NURSE Active Hospice Care is seeking Part Time and/or Per Diem compassionate and responsible Licensed Vocational Nurse. Under the direction of a Registered nurse, the Licensed Vocation Nurse provides skilled nursing and palliative care for the assigned hospice patients. He/she works cooperatively with the other members of the Interdisciplinary Hospice Team. The LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, and participates in the care plan development and implementation. Core Competencies: Demonstrated competency in performance of skilled nursing functions. Ability to work with terminally ill patients and their families. Compassionate attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.
