We offer great pay and great benefits POSITION SUMMARY The RN Supervisor will provide services in Atlanta to ensure and maintain the highest quality of care for our clients and customers, through our caregiver workforce, building and maintaining excellent relationships with all external clients, customers and field staff.

Responsible for optimal clinical and financial outcomes, as well as stakeholder satisfaction for a caseload of clients.

Responsible for assessing clients, developing a plan of care and addressing/resolving any ongoing clinical issues.

JOB RESPONSIBILITIES Clinical Supervision Initiates and documents an initial assessment or reassessment of the client for starts or restarts of care and document in the EMR.

Assesses a client, determines necessary and needed interventions while collaborating with the family.

Provides supervision for clients’ clinical care, ensuring appropriate care is delivered according to the appropriate scope of practice.

Works with field caregivers and nurses to ensure full understanding of the clients’ case requirements and how to successfully work with the clients.

Determines goals of clients and families to ensure that day-to-day care supports those goals.

Prepares reports for Practitioner/obtains practitioner orders for care.

Communicates to assigned field caregivers new or changes in client care.

Prepares and maintains timely and accurate reports and schedules for external care management stakeholders ensuring that they are up to date.

Documentation Provides prompt communication and documentation of changes in client medical status/emergencies to appropriate team members, family and practitioners as necessary.

Maintains EMR to ensure timely accurate processes are followed in compliance with regulatory bodies.

Conducts and documents assessment and visits in accordance with regulatory compliance.

Provides accurate and timely disbursement of client information to internal and external staff.

Ensures appropriate client documentation in the home is available/maintained (electronic and manual).

Education Provides continuing education and communication with clients and family members to ensure they are accurately informed of clients’ needs Identifies caregiver training needs and opportunities to improve care delivery.

Includes coaching and counseling to the highest level of performance possible.

Oversight Provides oversight of caregivers assigned to caseload.

Ensures caregivers job related needs are met in a timely and supportive manner.

Conduct start of care (SOC) visits and CNA supervisory visits.

Collaborates with Care Coordinators with detailed instructions for client clinical and staffing needs to be met.

Provides supervision of clinical activities for PRN Assessment nurses, including patient assessments, care plan development and implementation, on-site field visits, and the continual evaluation of effectiveness of the care plan to optimize the client’s health and independence.

Assist with hiring of additional nursing staff as needed.

Other duties as assigned.

REQUIRED EXPERIENCE, EDUCATION, SKILLS & ABILITIES Graduate of an accredited RN program, with current active unrestricted nursing license.

CPR certification required.

A minimum of 5 years nursing experience require.

A minimum of 3 years home care experience.

At least 1 year experience working with TBI and/or SCI patients.

Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and know whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.

Ability to communicate and coordinate effectively and professionally with internal leadership, field caregivers, external stakeholders, medical practitioners and case managers.

Knowledge of head to toe nursing assessment for clinical decision making/problem solving.

Knowledgeable of the signs of emergent situations and interventions required for individual clients.

High level of critical thinking and problem-solving skills.

Ability to work in a fast-pace environment; always maintain professional demeanor and appearance.

Bilingual Spanish speaking is a plus
