Overview
The Community Health Department at Dignity Health California Hospital Medical Center regularly performs health screenings in the community. Our free health screenings are conducted at various churches, schools, community centers, and other organizations dedicated to improving the quality of life for our sisters and brothers who are underserved and disenfranchised.
The Community Health Department serves as an advocate for those individuals who are uninsured and without medical care. We screen for early detection of disease and link participants to medical care and services at a medical home. These services are a valuable part of increasing health and wellness within the community and strengthen our healing ministry of Jesus.
Founded in 1887, Dignity Health – California Hospital Medical Center is a 318-bed, acute care, nonprofit hospital located in downtown Los Angeles. Responsibilities
About this opportunity:
- The candidate needs a comprehensive understanding of nursing science and the ability to apply this knowledge in planning, implementing and evaluating patient care in the community setting. Responsible for quality of nursing care and related documentation. Includes a thorough knowledge of standard equipment and techniques to assist/ treat patients. Health screenings include (height, weight, BMI, waist circumference, blood pressure, blood glucose, total cholesterol, hemoglobin, hemoglobin A1c)
- Candidate works independently within established professional and agency standards, regulations, and guidelines to apply a range of technical, interpersonal, analytical, and organizational knowledge and skills to problems of health as these affect the individual, family, and community. Involves making independent judgments in assessing health status, formulating a nursing diagnosis, identifying health improvement outcomes, developing and implementing strategies and activities, and providing and evaluating nursing services to ensure that outcomes are achieved.
- Provides a variety of nursing field-based services in various settings (schools, churches, community centers, etc…) to patients and their families; recognizes multi-cultural practices and beliefs, and is sensitive to their impact on health care, promotion and maintenance.
- Provides input into the development, revision and implementation of protocols, policies and standard procedures and to the identification of program goals, objectives and changes.
- Responds to and researches inquiries from patients, the public and other health care providers to explain program services offered; relays and interprets test results, makes referrals, and coordinates care; prepares teaching aids, delivers lectures and performs demonstrations
- Documents information during initial and ongoing assessments in accordance with policies and procedures.
- Provides public health education and training to various groups through public speaking, multimedia presentations, and written materials to explain the function of public health and its roles and services in order to improve individual, family, and community health outcomes and encourage community members to become more engaged in their own healthcare.
Qualifications
Qualifications:
- Minimum three (3+) years of acute care nursing or 2 years of public health nursing experience.
- AA or BS in Nursing.
- Bilingual English/Spanish speaking skills required.
- Ability to conduct individualized patient education and community education.
- Requires the ability to exchange pertinent non-routine information, answer questions and offer guidance, either face-to-face, through written correspondence, or via the telephone, with individuals outside the organization on a continual basis.
- Requires tact and some persuasion beyond the levels of courtesy and politeness required for all jobs.
- Requires the ability to function independently in a community setting.
- Current Registered Nurse License issued by the State of California.
- BLS card
- Possession of a valid California driver’s license and current auto insurance.
