Pay: $90,000-105,000 plus 5k Sign On Bonus!

The Neogen Care Clinical Supervisor – Home Health directly supports the Director of Patient Care Services (DPCS) in leading the development of all Neogen Care home care service capabilities and the overall direction of home health clinical services. The Clinical supervisor helps the DPCS to establish, implement, and evaluate processes, procedures, goals, objectives, and quality assurance for Neogen Care home health services to meet and promote organization vision, mission, philosophy, and quality standards.

ESSENTIAL FUNCTIONS: Under the direction of the DPCS:

  • Coordinate and lead direct and indirect patient care services provided by Neogen Care clinical personnel within their assigned area.
  • Provide guidance to and counsel clinicians and therapists in their continually improving all aspects of home health care services.
  • Lead and manage clinical teams.
  • Assess, plan, implement, and evaluate patient and family/caregiver care for Neogen Care clinical personnel in their assigned area.
  • Evaluating the performance of Clinical and Nursing Supervisors and field clinicians and therapists to implementing corrective measures to optimize their performance.
  • Conduct annual clinical performance evaluations, or more frequently if indicated, initial critical skills verification, and annual skills verification.
  • Train clinicians to ensure the proper and accurate submission of patient care charts via an electronic charting system (Kinnser) within CHAP and HMO client requirements.
  • Participate in orientation and training of new organization personnel, to include Critical Skills Checks, EMR training, and clinical training.
  • Plan and conduct in-service training and field training.
  • Conduct required field supervisory visits to evaluate and monitor field clinicians performance and assess skills.
  • Assist with the evaluation of organization performance via performance improvement programs, productivity evaluations, and quarterly and annual reviews. Assure the quality and safe delivery of home health services provided by Neogen Care.
  • Assure compliance with local, state, and federal laws regarding licensure and certification of organization personnel and maintaining compliance with CHAP Home Care standards.
  • Providing oversight of patient care services and personnel in their assigned area to include the scheduling of patient and personnel assignments; coordinating patient care; coordinating referrals; assuring patient needs are continually assessed; and assuring the development, implementation, and updates of individualized plans of care.
  • Be available on call one weekend per month on a schedule to take calls and handle or elevate emergent patient care issues.
  • Through overall efforts to train and mentor clinicians, prevent patient complaints. Support responses with short term and long-term plans for resolution.
  • Closely follow QAPI programs to completion.
  • Other duties as assigned by the Chief Executive Officer.

Required Education And Experience

  • Must be a Registered nurse (RN) with current licensure to practice in the state of California.
  • Minimum of Associates Degree in Nursing. Undergraduate (BSN) degree in Nursing from a program accredited by the National League for Nursing and graduate degree (health care) a plus.
  • Minimum of one (1) years’ supervisory experience in the health care field within the last five (5) years.
  • Knowledge and expertise in clinical decision-making for the patient population.
  • An understanding of Quality Assessment reviews.
  • Demonstrated ability to effectively supervise and direct professional personnel.
  • Excellent observation and verbal and written communication skills.
  • Excellent customer service and interpersonal skills.
  • Ability to work with high degree of precision and attention to detail to achieve accurate and timely delivery of services and to follow up and close out all open cases.
  • Ability to work efficiently with typical office computer software programs (MSWord, Email, etc.); and ability to efficiently use an electronic health care charting system.
