Assume responsibility for a patient and family, including, assessing, planning, implementing, and evaluating care.

Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and family Maintain regular communication with TD, to review care.

Maintain regular communication with the attending physician concerning patient and family care.

Obtain data on physical, psychological, social and spiritual factors which may influence patient and family health status and incorporate data into the plan of care.

Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated, changes are reflected as they occur, and all documentation is completed before leaving the IPC at the end of the shift Accept responsibility for coordinating the physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, and by providing direct care Instruct the patient and caregiver how to; Administer medication and recognize side effects.

Perform personal care and oral hygiene.

Assist in lifting, moving, ambulation and exercise.

Prepare for the events preceding death Inform TD, of unusual or potentially problematic patient and family issues Attend Hospice team meetings and other patient conferences as deemed necessary by the TD.

Provide appropriate support at time of death Share in providing 24/7 coverage to patients and families.

Assist in making hospice aide assignments, prepare written instructions for the aide and supervise the aides in the home.

Participate in hospice orientation and ongoing education programs QualificationsHospice Nurse Qualifications: Associates degree, preferred Registered Nurse license required in state(s) practicing Current and valid CPR certification, preferred
– required in Georgia & California Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing, preferred Minimum one year experience required in Connecticut Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance AccentCare Job ID 6860-en-us.
