The Multipurpose Senior Service Program (MSSP) Registered Nurse (RN) conducts Assessments, Health Review visits and certifies the Level of Care to ensure participants meet MSSP program eligibility. RN collaborates with Care Managers, participants, and their families to develop clients’ individualized person-centered plan of care. The RN is responsible for performing face-to-face assessments with older adults in their homes. Topics of assessment include diagnoses, medications, assistive/sensory devices, systems review, ADLs/IADLs (Activities of Daily Living and Instrumental Activities of Daily Living or basic self-care tasks). The RN will gather all necessary information to determine whether the individual meets the Level of Care to enroll in the MSSP program. The RN will assess the client as a whole individual and be responsible for following up on any crisis-related issues or medical concerns as needed. The RN will collaborate and consult with physicians and other health professionals to provide advocacy, order medical supplies, and Durable Medical Equipment (DME) as needed.

Qualifications:                                  

  • Current RN License; must be active and unrestricted for the State of California
  • Must have a reliable car, proof of auto insurance, and valid unrestricted CA Driver’s License
  • Ability to speak Russian, Spanish, or Farsi desired
  • Minimum of one year of clinical nursing experience
  • Experience working with the older adults preferred
  • Community-based service delivery experience preferred
  • Ability to function independently and with flexibility
  • Experience with Electronic Health Records systems desired
  • Proficient with Microsoft Office suite (Outlook, Word, Excel)
  • Demonstrated ability to work in an interdisciplinary community setting maintaining appropriate professional boundaries
  • Culturally sensitive and non-judgmental in working with a multicultural client population
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

 Responsibilities/Essential Functions:

  • Responsible for conducting in-depth face-to-face Assessments covering medical, health, and rehabilitation concerns
  • Responsible for certifying Level of Care, conducting Health Reviews, and medication reviews
  • Responsible for ordering medical supplies and requesting Durable Medical Equipment (DME) as needed
  • Collaborate and consult with physicians and other health professionals
  • Coordinate care for MSSP participants and collaborate with MSSP care management staff and Supervisors
  • Collaborate with MSSP participants, Care Managers, and their families to develop clients’ individualized person-centered care plans
  • Comprehensive and timely documentation of all CDA required forms in the Electronic Health Record System
  • Other duties as assigned
