Description:
Under general supervision, provides patient education, support, and interventions via telephone and/or video to patients who have Remote Patient Monitoring equipment in the home. Uses a process of nursing intake, assessment, planning, and care in accordance with physician orders, the established plan of care, the patient’s diagnosis, policies and procedures, and the Nurse Practice Act.
The focus of the position is:
To ensure quality patient care across the continuum, reduce avoidable readmissions, and improve outcomes for the highest risk individuals with multiple chronic conditions (specifically diabetes and hypertension); including those challenged with COVID-19.
To provide high level nursing care through the ability to perform phone triage and intervene or escalate appropriately using critical thinking skills for complex adult and/or geriatric patient populations.
To ensure appropriate screening of patients in need of continuum integration services including but not limited to remote patient monitoring and telehealth virtual visits.
To provide direct patient education and consultation on continuum integration services as needed.
Duties and Responsibilities Essential Accountabilities:
Helps to identify patients who are appropriate for RPM. Establishes vital sign parameters, obtains physician orders, develops a plan of care, and documents findings. Develops and maintains rapport with patients and caregivers via phone and/or video communication. Assesses and evaluates patient data and needs; providing appropriate support. IE: Patient/family health education, direction, and intervention.
Notifies physician (clinical lead escalation) of changes in patient condition or status via critical alerts in platform solution and sends reports (data trend/device readings), as needed. Identifies the need for and recommends appropriate disciplines and community resources for optimum patient care.
Maintains clinical skills within nursing practice norms and participates in group projects, training, and education as needed. Informs team lead of perceived problem areas related to program, patient care, or community relations.
Demonstrates a professional attitude, sound judgment and continuity in interactions with physicians, other health care providers, and community agencies. Promotes a positive corporate image. Acquires and utilizes knowledge of community resources. Demonstrates clinical, interpersonal and communication skills. Develops computer skills required.
Responsible for completion of all required documentation within 24 hours. This must include any changes in condition and a follow-up with the provider or other disciplines, as appropriate, with the supporting documentation with the platform patient record.
Attends/participates in required RPM meetings with client and staff.
Common Expectations:
Functions as a member of an inter-disciplinary team, encouraging and maintaining open communications. Provides clinical supervision to the RPM medical assistant and device management specialist for input regarding his/her performance to the appropriate manager. Assists in the orientation of new staff or continuing education of existing staff as requested. Seeks consultation and direction from management personnel as appropriate.
Qualifications Minimum Experience:
1 – 2 years; recent acute or urgent care experience
Minimum Education: Nursing Diploma or Associates Degree in Nursing.
Preferred Experience – Critical or Urgent care experience, EMR
Preferred Education: Bachelors of Science, Nursing (BSN)
Required Certification: Registration and current RN license in Arkansas or eligible or compact state.
