The Department of Cardiology and Heart Institute are expanding their outpatient clinic practice to offer post-discharge evaluations and treatment to CSMC Medicare patients with Heart Failure who are enrolled in the Bundled Payment Cardiac Care clinical episode. You will care for patients and treat accordingly to avoid admission to the medical center and a 30-day readmission. You will also triage patients from the clinic to other treatment sites (e.g. Procedure Center or other Clinics) as necessary.
Summary of Essential Duties:
- As a Clinical Nurse III you will perform a variety of direct patient care activities specific to the unit and needs of the patient population
- Perform patient assessments, triage patient concerns, provide clinical education to patients for pre-ops and other disease management education
- Manage anticoagulation, coordinate patient care, and maintain overall management of the ambulatory cardiac clinic with over 20 providers daily
- Obtain prior authorizations
- Triage phone calls from patients, pharmacies and other providers
- Identify the ability of pediatric, adolescent, adult and geriatric patients to follow direction/instructions, as well as safety concerns in the age-related populations
- Institute additional measures to provide a safe environment
