Job Description Currently seeking a registered nurse for field nursing visits:
– Provides complete initial assessment for hospice admission including an explanation of hospice benefit
– Coordinates the hospice plan of care with members of the care provider team
– Performs/coordinates on-going case management/supervisory visits according to patient’s needs
– Provides education of the client and family regarding the disease process, self-care techniques, and disease prevention strategies, as appropriate
– part-time or full-time Qualifications: Licensed in the State of California by the Board of Registered Nursing (RN).
Current CPR card, health clearance, and clinical skills self-assessment Minimum of one year of professional nursing experience.
