Job Description Job Title Registered Nurse Job Summary To plan, coordinate and provide hospice care in accordance with the patients plan of care General Accountabilities Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met. Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis Consult with the attending physician and and Hospice Medical Director concerning changes in patients condition requiring order changes and or changes in the plan of care Submit patient visit schedule weekly Take responsibility for ensuring patients plan of care are kept current Provide education to patients/family members regarding the dying process, plan of care and offers guidance with health care decisions Conduct an initial as well as ongoing comprehensive assessments of the patients Conduct a physical examination of the patient, including vital signs, physical assessment, mental status, appetite, diet, etc Evaluate the patients environment to determine what assistance will be available from family members in caring for the patient and what services may be needed Explain nursing and other agency services to patients and families as a part of planning for care Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the comprehensive plan of care Supervise and evaluate the care given by the Hospice Aide at a minimum of once every 14 days and as needed Provide guidance and supervision to the LVN and supervises LVN a minimum of every 30 days Cooperate with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan Prepare care plan for Hospice Aide Job Qualifications Must be graduate from an accredited School of Nursing Must be licensed in the State as a Registered Nurse Must have excellent assessment skills and problem solving skills Should be skillfull in organization and in the principles of time management Must be a licensed driver with an insured automobile in good working order The ability to communicate well, both verbally and in writing Skills Excellent verbal and written communication Service orienation Active listening Monitoring Coordination Social perceptiveness
