Unifour Family Practice is seeking a Registered Nurse to fill a newly created position within our practice.
We are a privately-owned, Christian family practice with a dedicated team committed to providing comprehensive healthcare to the local community in Hickory and surrounding Unifour area.

Candidate will have an array of responsibilities that will contribute to the overall well-being of our patients.
Primary Duties and Responsibilities:
•Work directly with a physician to oversee Annual Wellness Visits for Medicare patients.
•Support and coordinate office based primary care including screenings, history, and transitions of care.
•Develop appropriate care plans and be a champion of chronic care management.
•Communicate and collaborate with the medical staff team to achieve patients’ goals.
•Perform other nursing duties as assigned including pediatric, gyn, and minor procedures.Required Qualifications:
•Hold a current RN license in the state of North Carolina.
•Be a compassionate partner for the patients and ensure they are fully informed and capable of self-care responsibilities outside of the practice.
Preferred Qualifications:
•Experience in documenting clinical data in an electronic health record system (EHR).
•eClinicalWorks experience is a plus.
•Clinical experience preferred.Knowledge, Skills, and Abilities:
•Excellent interpersonal and communication skills, both written and oral.
•Be motivated, independent, and ability to multi-task.
•Be a compassionate team player wanting to improve the health and well-being of all patients.
