POSITION SUMMARYThis position requires monitoring and reaching out to patients to improve clinical performance measures. The Population Health Coordinator will run reports and research data on patients with diseases and preventive care measures using Cerner, HealtheIntent, Business Objects and insurance portals. The Population Health Coordinator is responsible for answering incoming calls and calling outreach programs to schedule appointments; monitors patients until exams are completed; and uses Disease Registry, Cerner and Insurance Portals to reach out to patients who have not been seen by a Primary Care Physician. The Population Health Coordinator will remain up to date on all new Providers in the Medical Group, acts as a liaison for all patients who do not have an established PCP, and focuses on the marketing of new Providers.ESSENTIAL DUTIES AND RESPONSIBILITIES Runs weekly reports of patients that need to be contacted for Disease Management and Preventative Health compliance. Updates Cerner system with the data from Cerner and community physicians. Runs recall report on all patients who have not followed up on appointments. Tracks appointment availability for providers and reports to Manager of Population Health on new patients and physicals availability for Primary Care, Endocrinology, OB-GYN, and Rheumatology. Conducts outreach to patients with outstanding preventive care measures and disease measures; schedules appointments as needed and records appointment in Cerner. Monitors documentation of appointments in Cerner and follows up if appointment is missed. Works with Manager of Population Health to prioritize the needs of the Medical Group. Will not use or disclose individually identifiable health information of patients over and above that which is minimally required to perform assigned position responsibilities.POSITION REQUIREMENTSA. Education High School Diploma or GED Degree in appropriate field of study or equivalent work experienceB.
